Children in Need

Thanks to the hard work of our staff, the support of our Patients' Group (see link on right for more info) and the generosity of our patients we raised an excellent £342 .62 for Children in Need!  Well Done Carnewater!

If you live within our catchment area you can register to bkeyboard_stethoscopee a patient here at Carnewater Practice, and we look forward to welcoming you. 

 

 

Our catchment area covers most of Bodmin Moor, from the outskirts of Launceston to the east, Roche in the west, Sladesbridge and St Tudy in the north, and Lerryn and Lostwithiel in the south.  These are only given as a guide, so to check whether you can register with us please contact reception.

When you register, please bring your NHS Card with you—this is the simplest way to register.  If you do not have your card you will be required to complete a form , and to provide proof of your identity so that checks can be made to ensure you are entitled to free NHS treatment—please ask at reception for details of acceptable proof of identity.  This will also enable you to enrol for our on line services such as booking appointments and ordering repeat medication.

 

You will be asked to complete a New Patient Questionnaire - this will provide us with health information that we may need before your notes are forwarded from your previous practice.  As you will see there is also a on-line version of this form which you can send to us electronically - pleasefollow the link on the tab above.  If you are registering a family, a form will have to be completed for each family member individually.

 

You will be invited for a New Patient Health Check—during this a detailed history will be taken along with details of any medication that you use.  This will help us to provide a service to you until your notes are forwarded to us from you old surgery, so please make use of this opportunity.
